Mid West palliative care providers join forces to improve patient care

Care for Mid West patients receiving palliative care is set to improve with the launch of a comprehensive shared care patient record folder, a joint initiative between St John of God Hospital Geraldton’s hospice unit, WA Country Health Service Mid West and the Geraldton Mid West Palliative Care Service.
