Apply or make a referral

Making sure we’re the right fit.

First, we’ll need to get some details

That means we need to ask you fill in our referral form and then email it through to [email protected].

What happens once we’ve received a completed referral form

Once your completed referral form has been received, one of our staff members from the appropriate house will be in touch to discuss whether the program is a good fit. If it is, they will speak to the young person (in cases where they’ve been referred, the referrer is the initial point of contact) and then get details for references. At this point they will also be able to let you know whether there are any places currently available. 

Getting to know you

We meet all young people who have been identified as a suitable fit for our program face-to-face and conduct a reference check prior to offering a place in a Horizon House.

Once a place has been offered, we’ll invite the young person to come for dinner at the relevant house to meet the other young people living there.

Eligibility criteria for our general program and Broome house

Our program is for young people who are:

  • aged 16-22
  • homeless or at risk of homelessness
  • engaged, or committed to engaging within eight weeks of entry, in a daily activity program such as education, employment or other skills development 
  • young mums, must be engaged in a program as above, due to give birth within eight weeks or currently parenting
  • have capacity to engage with Support Coordinator and Youth Residential Caregivers to work towards personal goals and build independent living skills.  
  • be in receipt of a Centrelink allowance or other wage payment to contribute towards subsidised rent
  • not have engaged in significant substance misuse for at least three months
  • identified as low risk for aggression, suicidality or self-harm.

All St John of God Horizon House residents must be willing to work with us to obtain urgent ambulance cover.

Eligibility criteria for our Young Mother and Baby Program

Our Young Mother and Baby Program is for young people who are:

  • aged 16-22
  • homeless or at risk of homelessness
  • at least 12 weeks’ pregnant confirmed by a general practitioner (GP) or with baby
  • engaged, or committed to engaging within eight weeks of entry, in a daily activity program such as education, employment or other skills development OR due to give birth within eight weeks or currently parenting
  • have capacity to engage with Support Coordinator and Youth Residential Caregivers to work towards personal goals and build independent living skills  
  • be in receipt of a Centrelink allowance or other wage payment to contribute towards subsidised rent
  • not have engaged in significant substance misuse for at least three months
  • identified as low risk for aggression, suicidality or self-harm.
