Our rehabilitation at home program enables you to continue your recovery after injury, surgery or illness in the comfort of your own home.

Rehabilitation at home offers home visits from our senior rehabilitation clinicians, including:

• physiotherapists
• occupational therapists.

Our rehabilitation at home services are available to those who:

• have recently had orthopaedic surgery
• need to build up strength, endurance or balance due to cancer treatment, recurrent falls or an extended hospital stay.

Rehabilitation at home

Our rehabilitation at home service enhances patient's confidence in safely returning to their home environment after surgery, reducing anxiety and improving independence and quality of life.

Reconditioning at home

Our reconditioning pathway supports patients in the post-acute stage of their recovery, particularly for general medical or geriatric patients. This service is ideal once a basic level of mobility is achieved and inpatient rehabilitation is no longer necessary.

Occupational therapy at home

Pre-discharge assessment

Healthcare at Home provides pre-discharge assessments to support patients before a planned hospital admission and prior to returning home post-hospital stay. Our experienced team of occupational therapists reviews the home environment and recommends strategies and equipment to ensure it is safe for discharge. With the approval of the patient or family members, we also facilitate the installation of necessary equipment.

Post-discharge assessment

After hospital discharge, an occupational therapist can conduct a post-discharge assessment at the patient's home. This ensures the patient is using their equipment correctly and becoming increasingly independent in daily activities. Our service empowers patients to perform daily living activities independently whenever possible.

Rehabilitation at home is available in Western Australia across metropolitan Perth.

Fees

* Fees are subject to annual CPI.

At-home visits

• First visit (60 minutes): $189
• Follow-up visits (up to 45 minutes): $162

Telehealth appointments

• Per consultation (30 minutes): $98 

Payment options

You can pay by credit card over the phone.

You may be able to claim your visits or consultations through your private health insurance. Check with your private health fund regarding what rebates may apply.

DVA Gold and eligible DVA White cardholders can get this service fully funded by the Department of Veterans Affairs.

All cancellations within 24 hours are non-refundable.
