Accreditation
The Director of Medical Services is the primary point of contact
for any queries by medical, registrars in training, dental and
health practitioners.
Application forms can be obtained by
contacting the Director of Medical Services Executive
Assistant. All practitioners are required to attend an
interview and orientation to the hospital with the Director of
Medical Services. At this interview the application will be
reviewed along with the following documentation:
- Completed application form
- Credentials (primary, specialist degree where
relevant), original or certified copy.
- Current Certificate of Registration to
Practice in the State of Victoria, original or certified copy.
- Current indemnity insurance membership,
original or certified copy
- The names of three (3) professional
referees
- A current curriculum vitae
- A list of surgical procedures you wish to
perform and documentation verifying competency for each
procedure
- Documentation relating to Working with
Children check
It is also hospital policy to take an ID photo
that is to be worn whilst in the hospital. n ID photo will be
taken at the completion of the interview with the Director of
Medical Services.
The Director of Medical Services checks all
references. The Credentials Committee reviews applications
with recommendations forwarded to the Chief Executive Officer and
the hospital’s Medical Advisory Committee. Once approved,
accreditation is granted for a period of 12 months and reviewed on
expiry for a further (3) years.
