Referrers
Patients may either self-refer or be referred by their
General Practitioner or Specialist, who are given regular
updates on their patient's progress and who are notified when
services are completed.
You may refer yourself or your patient to St John of God
Health Choices by downloading our
referral form and faxing or emailing to us.
